Notice of Privacy Practices

At MECFS Clinic MN, we respect the confidentiality of your personal information, which is called the Protected Health Information (PHI).  That includes your name, address, phone number, medical diagnoses, results, etc. We take our obligation to keep your PHI secure and confidential seriously.

The Health Insurance Portability and Accountability Act (HIPAA) law requires us to provide you with this Notice informing you about how your medical information may be used and disclosed, the steps we take to protect your PHI, how you can get access to this information, and about your rights regarding your PHI.  Please review it carefully.

 

Understanding Your Rights 

Every time you interact with MECFS Clinic MN as a patient via a telehealth visit, a phone call or a portal message, a record is created. This is referred to as your medical record which serve as a legal document of your health, and a tool to communicate with other healthcare professionals.

Understanding what is in your record and how it is used ensures that the content is accurate, that you understand when others may access your information, which eventually helps you decide when to authorize disclosures to others.

Your Rights to Your Health Information Includes: 

  1. To obtain a copy of this notice by calling MECFS Clinic MN at 612-440-9699.

  2. To authorize the disclosure of your health information to another healthcare professionals. 

  3. To access your medical records. 

  4. To amend your health information and medical records that you believe is incorrect or incomplete.

  5. To request confidential communications in a specific way, e.g., at a certain mail address or phone number. 

  6. To limit or restrict what we disclose and who we disclose your health information to.  

  7. To receive an accounting of disclosures upon your request up to six years prior to the date of the request.

  8. To revoke your authorization at any time. We, however, are unable to take back any disclosure we have already made with your permission.

Our Responsibilities 

We follow the HIPAA and the Health Information Technology for Economic and Clinical Health (HITECH) laws to protect your PHI and will not use or disclose your health information without your authorization, except for the purpose of making a referral to an imaging study or lab work.

We reserve the right to change our policies and procedures to protect health information. If we make significant changes in how we use or disclose your health information, we will also change this notice and will provide you with an updated copy. 

​

Data Breach 

In the event of any breach of unsecured PHI, MECFS Clinic MN shall fully comply with the HIPAA/HITECH breach notification requirements, which will include notifying you of any breach may have impacted you and/or your family member(s), and actions we undertook to minimize the impact of the breach on you.
